Fourteen hospitals under the Ascension health care system have been awarded an "A" Hospital Safety Grade by The Leapfrog Group, a national nonprofit organization dedicated to patient safety. These grades signify Ascension's commitment to maintaining a secure environment for patients. Leapfrog's grading system, ranging from "A" to "F", evaluates hospitals on more than 22 performance measures, including accident and error prevention systems.
A significant percentage of Ascension facilities—nearly 78%—achieved either "A" or "B" grades. The average grade point among Ascension hospitals is 3.1, slightly above the national average of 3.0 set by Leapfrog.

- Ascension Via Christi Hospital in Manhattan (Manhattan, KS)
- Ascension Providence Hospital (Waco, TX)
- Ascension Seton Medical Center Austin (Austin, TX)
- Ascension Seton Hays (Kyle, TX)
- Ascension Seton Williamson Hospital (Round Rock, TX)
- Ascension Seton Northwest Hospital (Austin, TX)
- Ascension SE Wisconsin Hospital - Elmbrook (Brookfield, WI)
- Ascension NE Wisconsin - St. Elizabeth Hospital (Appleton, WI)
- Ascension SE Wisconsin Hospital - St. Joseph (Milwaukee, WI)
Nineteen other Ascension hospitals have attained a "B" grade, including Ascension Sacred Heart Emerald Coast (Miramar Beach, FL) and Ascension St. Vincent’s Riverside (Jacksonville, FL), among others.
